All coaches (head and assistant), and team
helpers are required to fill out and submit a Background Check form directly to
the S.P.O.R.T.S. office.
Accident Report

This form must be completed by the team coach
for each accident or injury sustained by a player on their team that occurs
during practice or a game. Please follow the instructions on the form. Please
forward a copy of the report to your league commissioners.

Program Evaluation
This year we would like to ask Parents and
Coaches to take a few minutes to complete an evaluation form to let us know how
we are doing. These evaluations are kept private and will be a big help to the
Cheer Board of Directors.
